There were 5 children observed in the child care space and 1 child in the bathroom. Child #1 was observed in the bathroom unsupervised with their hand in the toilet. Also, in the Infant Room, Staff Person #3 was in the closet with the door closed, leaving 4 children unsupervised. THIS IS CONTINUED NONCOMPLIANCE.
