The licensee had 3 children over the age of three who were moved to her connected license at the same home. She has duplicated child files at each location, but nothing on file that parents know. This must be in writing.

I was unaware I needed this form. I have had licensing inspect for 8 years. Nothing was said. The parents signed teh food program enrollment for both sites and they were aware their children would be using both day cares. We don't have filed trips. The daycares are in the same home.

At the time of the visit 4 children between 16 months and 3 years old were behind baby gate and fences. There were a total of five of these areas. Children must be able to move freely to play.

The main area has 5 centers with push button doors. The children can easily go in and out. They open the doors by themselves. The 16 month non walking child can open the door. I removed the doors. Children are able to move freely and play.
