|Findings: According to attendance and conversation with Director group including 1 or 2 toddlers: max froup size = 12, Center had 18 children on February 5.|
|Findings: Specialist observed several items transferred to secondary containers that were not labeled.|
|Findings: Licensing Specialist was unable to locate any staffing patterns. Director could not find any staffing patterns. Staffing patterns on file at Office of Child Care do not truly represent what is happening at the center. Children and staff are in different rooms then is reflected.|
|Findings: Sink in infant rooms is being used as a hand washing sink and food prep and toy cleaning sink.|
|Findings: Infants are not being taken out daily per conversation with staff member in infant room. Infant plans have not been created for the infants in care.|

|Findings: In a group containing 3 and 4 years olds, specialist witnessed staff leaving classroom. When the staff left the room, 13 children were left with one teacher. Ratio is 1:10.|
|Findings: Medication was found in infant room that did not have the appropriate forms signed by parent. Medication (non prescription) found in restroom was not labeled. Parental forms were not signed by parents.|

|Findings: A written contract was not found in the files of all enrolled children.|
|Findings: Medication in the infant room was not labeled with child's name, dosage or expiration date.|
