| Initial Comments: The following deficiencies were observed during the initial compliance inspection conducted on 8-19-2026, and are subject to change pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided during the inspection. 2 of 2 fingerprint clearance cards were verified as valid via the DPS website during the inspection. The following was discussed, but not limited to: 1. Staff 10-day training--must be completed within the timeframe. 2. First aid kit components--stock meds and gallon bags. 3. Liability insurance document—needs to reflect the change from 7 to 10 (total capacity). 4. Outdoor activity area---4” gap under fence (in process of being filled). 5. Fridge/freezer temperature requirements. 6. Storage and accessibility of cleaning equipment--must be inaccessible. 7. Children’s sign-in and out records. 8. Children's Emergency Information and Immunization records and telephone authorization code. 9. Staff attendance records. 10. Accessibility of drinking water. 11. Monthly fire drill documentation. 12. Classroom rosters. 13. Notification of pesticides. 14. Child abuse and neglect--mandated reporter. 15. Written approval for personal products supplied by the facility. 16. Children with special needs and in the individualized plan. 17. Discipline and guidance procedures. 18. Written accident, evacuation, and emergency plans. 19. Notifying parents and documenting injuries. 20. Illness and infestation log. 21. Medication procedures. 22. Transportation and Field Trip Procedures. 23.
