Frenchs Day Care - Lake Ariel PA Family Child Care Home

Noncompliance Area: While at the facility unannounced on 8/18/20 for the complaint visit the inspector observed child #1 in a portable napper/rocker. The child was not strapped in and the inspector could find no documentation of a medical reason that would allow for the portable napper/rocker to be utilized.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: While at the facility unannounced on 8/18/20 for the complaint visit the inspector observed staff #1 and #2 changing a diaper. Staff #2 bagged the diaper in a separate bag and removed the diaper from the facility bypassing the hands free covered can.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: While at the facility unannounced on 8/18/20 for the complaint visit the inspector observed staff #1 and #2 changing a diaper. Staff #2 did not wash their hands following the diaper change.

Correction Required: A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child.

Noncompliance Area: While at the facility unannounced on 8/18/20 for the complaint visit the inspector found a baby bottle on the counter that was not labeled with a child's name.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: While at the facility unannounced on 8/18/20 the inspector found staff #1 and #2 to be without facial coverings.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Face masks must be worn by staff as described in the revised announcement dated 7/8/20 of the Order of the Secretary of the Pennsylvania Department of Health Order for Universal Face Coverings.

Noncompliance Area: While at the facility unannounced on 8/18/20 for the complaint visit the inspector could not find a file for child #1 and child #2. Neither staff #1 nor staff #2 could locate a file for either of these two children when asked to do so.

Correction Required: An operator shall establish and maintain an individual record for each child enrolled in the facility.

Noncompliance Area: While at the facility unannounced on 8/18/20 for the complaint visit the inspector found staff #1 and #2 to be working in direct child care with nine children and neither persons had a staff file as is required by regulation, nor any health assessments or tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Correction Required: A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: While at the facility unannounced on 8/18/20 for the complaint visit the inspector found staff #1 and #2 to be working in direct child care with nine children and neither persons had the required PA State Police clearance, FBI clearance, Childline Clearance and NSOR as is required by regulation.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). The legal entity must arrange for themselves to receive PQAS approved training regarding the hiring staff and the PA child protective service law (CPSL). It does not matter if the PQAS approved training is obtained through the Regional Key. The legal entity must receive DHS approval of the training content prior to scheduling the training. This training will not be allowed to count towards the biannual 12 hours of training required by regulation.

Noncompliance Area: While at the facility unannounced on 8/18/20 for the complaint visit the inspector found staff #1 and #2 to be working in direct child care with nine children and neither persons had proof of first aid competence.

Correction Required: One or more persons competent in first-aid techniques shall be at the facility when day care children are in care.

Noncompliance Area: While at the facility unannounced on 8/18/20 for the complaint visit the inspector found nine children in care, which exceed the six children unrelated to the operator allowed by regulation.

Correction Required: The number of children in care may not exceed six children at any one time who are unrelated to the operator. The operator must arrange to receive a minimum of two hours of training dealing with ratio of children. The operator must receive DHS approval of the training content prior to scheduling the training. Further, this training will not be allowed to count towards the bi annual 12 hours of training required by regulation. The operator will be expected to demonstrate their knowledge and ability to effectively ensure that the ratio of children is maintained at all times during several unannounced visits made by the inspector.

Noncompliance Area: While at the facility unannounced on 8/18/20 for the complaint visit the inspector observed a bottle of 409 cleaner, baking soda and a banana, out on the counter, sharing the same space.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: On 3/21/19 the inspector observed staff #1 place a child's bottle into the microwave for heating.

Correction Required: Bottled formula may not be heated in a microwave oven.

Noncompliance Area: While at the facility on 3/21/19 the inspector observed an aerosol can of Pledge and a bottle of Gold Bond hand lotion out and within reach of children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.
