Plan: I will address the staff and implement a new system for taking the children to the bathroom to ensure that supervision is maintained at all times by staff in that classroom. Supervision will be maintained at all times while children are using the bathrooms and the program takes it seriously.

Supervision means the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Licensing observed two children, one 3 year old and one 4 year old unsupervised and without staff present in the boy's bathroom. Licensing also observed a 4- year coming out of the girls bathroom unsupervised and without staff present. The boys and girls bathroom's are located adjacent and across the hallway to the 3's and 4's classroom. During the time the boys were unsupervised licensing observed the 3's and 4's teacher in the classroom sweeping at the other end of the classroom therefore the teacher was not able to observe or oversee the children in the bathroom. All three children are in the 3's and 4's classroom.

Licensing observed both out-door play areas did not have loose-fill materials that meet licensing requirements, at a depth of at least six inches surrounding equipment that is taller than 4 ft.
