Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 3/11/2026 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Notice of Inspection Rights was provided to the licensee at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but are not limited to: *Ensure children are only in licensed areas. *Ensure required documents are completed before the starting date of employment. *Ensure each room has the required documents posted. *Ensure classrooms have a sufficient amount of books accessible to children. *Ensure screen time is on the lesson plan in minutes. *Ensure the freezer that contains items for enrolled children has a thermometer. *Ensure emergency record cards are complete.

There were 3 staff files reviewed. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid at the time of the inspection. Please submit the Emergency Disaster Contact Form via email. The following was discussed, but not limited to: *Please ensure fire inspection reports are violation free. *Please ensure Central Registry background requirements are submitted through the appropriate portal. *Please ensure the soiled diaper containers are maintained with tight fitting lids. *Please ensure rugs/carpets are not rolled up along the corner edges.

Compliance Officer #1: Celeste Angulo Bureau Chief: Margaret Bernal Please submit the Written Documentation of Corrections within 10 days of receipt of this Statement of Deficiencies. 2 of 2 Fingerprint Clearance Cards were verified through the DPS website during the inspection. The Empower Survey was emailed to Facility Director. The following was discussed but no limited to: **Diaper changing area staying clear of non-diapering items. **Outdoor areas kept clear of hazards.
