Within two weeks after a new staff member begins working at the center, the sponsor or sponsor representative shall submit to the Department a completed CARI consent form for the new staff member.

I have used this provider for more than 6 months

Horrible. My child was locked in the owners off with her dog and bitten. If you do not send snacks WATER included there's a 90% you'll pick up a cranky and hungry child.
