Violation Observed: Ensure children are supervised at all times. Staff in Rooms A & B repeatedly walked out of the classroom leaving the children unsupervised.

The center shall be permitted to obtain a valid fire safety inspection certificate issued by the municipality in which it is located, based on a fire inspection conducted within the preceding 12 months, and submit a copy of the certificate to the Office of Licensing in lieu of a CO or CCO, if the center serves only children 2 ½ years of age or older and is located in a public school building that is used as a public school.

Violation Observed: Ensure the fire protective system is operable at all times. At the time of inspection, the approved fire panel was in trouble mode, reading "no battery". The center is using a "home smart" monitoring system which is not approved by the local fire official. The center must reinstate a properly licensed, commercial grade monitoring system.

The director or his or her designee(s) shall be on the premises at all times when the center is operating and the designee shall not have full-time classroom responsibilities during the times when the director is not present at the center.

Violation Observed: 82. Ensure that the director or director designee does not have full time responsibilities in the classroom. The director designee was required in the infant room to maintain ratios but needed to break ratios and work in the kitchen to prepare lunch for the children. | 2/20/20 - Ensure the director or designee is present at all times. At the time of inspection, both the director and designee were not present and staff were not aware of who was in charge.

2019-12-16 2020-01-10 3A:52-4.10(b)(1) Child Abuse Record Information background check procedures
When the center applies for a new or renewal license or Certificate of Life/Safety Approval, the sponsor or sponsor representative shall submit to the Department the completed CARI consent forms for all staff members who are or will be working at the center on a regularly scheduled basis. Within two weeks after a new staff member begins working at the center, the sponsor or sponsor representative shall submit to the Department a completed CARI consent form for the new staff member.

Violation Observed: 90. Renewal CARI clearance is pending for two staff and two new staff.
2019-11-13 2019-12-16 3A:52-6.2(a)(2) Program equipment for children's daily activities
The center shall provide a supply of age-appropriate and developmentally appropriate program equipment including play equipment, child-size furniture and supplies that are non-toxic and safe for use by children in the age groups served.

Violation Observed: 17. Children in all classrooms were not provided with a variety of age-appropriate activities as required. Children in room #5 ranged in age from fourteen-months to three years-old and materials and activities provided were not age-appropriate.
