Initial Comments: There were no deficiencies observed at the time of the Compliance Inspection conducted on 4/2/2026, subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility at the time of the inspection. 2 of 2 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Emergency Disaster Plan update form was completed at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation link to the Provider.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 04/08/2025 and are subject to changes pending programmatic review. Please complete the Written Documentation of Corrections through the Licensing Portal within 10 days. 3 of 3 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was emailed at the time of the inspection. The Emergency Disaster Plan Update Form was emailed at the time of the inspection. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. The following was discussed but not limited to: 1. The room capacity and emergency evacuation plans must be posted in the Multipurpose Room (MPR). 2. The outlets on the stairs leading up to the stage in the MPR must be covered. 3. Paper towels are available in toilet rooms. 4. Medication forms must contain specific reasons to administer medications. 5. Staff attendance must include times of arrival and times of departure.

Initial Comments: There were no deficiencies observed at the time of the Compliance Inspection conducted on 4/10/24. Compliance Officer #1: Jennifer Forschino A full inspection was conducted at this time. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.
