| Findings: LS conducted a complaint investigation on today's date. LS interviewed provider, who stated that parent came to her home on January 18, 2024 around 11 AM demanding that she take her child, but the parent had no medical documentation for the child, so the provider stated that she told the parent she would need medical documents before accepting the child. The provider stated that the parent cursed at her and left. The provider stated that around 2 PM, the parent came back and asked the provider to sign a paper for scholarship. The provider stated that she didn't have her glasses on, and while she was looking for her glasses the husband walked to the door and the parent spit in his face. The provider stated that her husband closed and locked the door and the parent began kicking and banging the door. The provider stated that she opened the door again to sign the paper and the parent forced themselves in the house and jumped on the provider and assaulted her. The provider stated that there were 5 children present in the room as this was happening and they were all scared and crying. The parent then ran out and the provider called the police. | |||
