When children in space #3 were transitioning to the bathroom, they went down the hall and around the corner where the teacher was unable to see them walking into the bathroom on two occasions. A child enrolled in space #4 opened the door to the lobby and left the classroom space unattended.

In the outdoor spaces, the grass was overgrown along the fences and around stationary equipment. Also, there were wooden tree stumps that had fungi growing on them.

In space #1, three (3) tubes of Desitin were on file for children that were no longer enrolled. In space #2, a tube of Desitin was on file for a child that was no longer enrolled.
