|Findings: a) Surveyor reviewed the statement of services and observed that the disenrollment procedures, and a statement that a parent has access to the areas on the facility premises where the parent's enrolled child is receiving child care services was not included. b) The Statement of Services also states that medical or accident insurance is not provided.|
|Findings: a) Surveyor reviewed the children's attendance records and observed the following: b) Child 1 - was not signed in with a time or signature on 1/8/19, and was signed out with only a time on the same day; no signature. c) Child 2 - did not have a time out on 1/7/19 and 1/8/19; only a signature. d) Child 3 - was not signed out with a time or signature on 1/8/19. e) Child 4 - was signed out with a time only on 1/8/19; no signature.|

Staff 1 did not have three credit hours or more in early childhood, child development, or a closely-related field from an accredited college or university, or at least 30 actual hours of instruction, provided in conferences, seminars, lectures, or workshops in early childhood, child development, or a closely-related field. Therefore, is not qualified to be the Director Designee.|
