Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Prescribed medication was not administered according to the prescription, using the amount and frequency of dosage specified on the label. A child in space 4 has been prescribed a medication requiring the medication be given to the child 3x daily before meals and 4x daily before snack. The medication log documenting when the medication was administered for this child was last updated on 10/6/22.

Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. A child enrolled in space 4 and a child enrolled in space 5 have prescribed medication for a chronic illnesses. Both document a 1-year authorization to administer the medication. Authorization periods for administering emergency medications such as epi-pens and Albuterol are only valid for six months at a time.
