LATER ON THE SAME DAY 9/15/20 CHILD #1 LEFT OUT OF THE CLASSROOM WHILE STAFF#2 WAS TENDING TO ANOTHER DAYCARE CHILD. CHILD#1 WALKED PASSED THE FRONT DESK OUT THE DOOR, CHILD#1 WALKED DOWN A NARROW SIDEWALK OF A BUSY TWO LANE STREET. HE WAS FOUND TWO BUILDINGS AWAY BY A PASSERBY WHO CONTACTED THE POLICE. CHILD#1 WAS FOUND A BLOCK AWAY FROM A CONSTRUCTION LOCATION. STAFF#1 AND STAFF#2 DID NOT REALIZE CHILD#1 WAS MISSING UNTIL THEY WAS RETURN BY THE LOCAL POLICE DEPARTMENT. THE DAY OF COMPLAINT VISIT 9/18/20 OBSERVED CHILD#3 IN THE HALLWAY ALONE. STAFF#3 STATED THEY WERE ASSISTING A DAYCARE CHILD IN THE BATHROOM WHEN CHILD#3 STEP OUT IN THE HALLWAY ALONE WITH NO SUPERVISION.

Correction Required: Children must be supervised at all times. The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for counting children is followed at all times e.g., a checklist. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format.
