Noncompliance Area: At the time of the unannounced inspection on 11/20/25, it was found that staff #1 grabbed child #1 and tapped child on the arm.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child.

Noncompliance Area: At the time of the renewal inspection, staff #1 who was identified as the director, did not have a complete file. File did not contain a staff health assessment and two written non-family references.

Correction Required: An individual record is required for each facility person. A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person.

Description: Age and Training

Noncompliance Area: At the time of the renewal inspection, staff 6, 9, 12, 13, and 14 did not have proof of 12 hours of child care training.

Correction Required: A staff person shall obtain an annual minimum of 12 clock hours of child care training.

Description: Fire safety - 1 yr.

Noncompliance Area: At the time of the renewal inspection, staff 1, 2, 8, 11, and 12 did not have current fire safety training.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Description: Health and Safety Training

Noncompliance Area: At the time of the renewal inspection, staff 1, 2, and 12 need the revised health & safety training. Staff 4, 8, and 10 need the updated health & safety training.

Correction Required: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10.

Description: Health and Safety Training - Pediatric First Aid and CPR

Noncompliance Area: At the time of the renewal inspection, staff 1, 8, and 12 did not have proof of Pediatric First Aid & CPR.

Correction Required: Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire.

Description: Comply with CPSL/CPSL information

Noncompliance Area: At the time of the renewal inspection, the file of staff #1, who was identified as the director, did not contain required clearances under CPSL, PA State Police, PA Child Abuse, FBI, NSOR, and NJ Clearances. The mandated reporter training was also missing from the staff file.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1 may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person # 1 may not work in a child care position at the facility.

Description: Employed and present 30 hrs/wk

Noncompliance Area: At the time of the renewal inspection the facility did not have a qualified director employed at least 30 hours per week.

Correction Required: A director shall be employed by a facility and be present at the facility site a minimum of 30 hours per week.

Description: Age and Training

Noncompliance Area: On the day of visit observed the files for staff#1 who have 3 hours of childcare training in the files. Staff#2 only have 6 childcare trainings in the files.

Correction Required: A staff person shall obtain an annual minimum of 12 clock hours of child care training.

Description: Aide qualifications/Exp, educ., training prior to facility

Noncompliance Area: On the day of observing the files staff#2 was missing proof of education.

Correction Required: An aide shall have attained one of the qualification levels specified in §3270.37(b)(1)-(3). A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: ON 9/8/20 CHILD#1 RAN OUT THE CLASSROOM PASS THE FRONT DESK WHERE STAFF#1 WAS SITTING. CHILD#1 WAS ABLE TO MAKE IT TO THE PARKING LOT WHERE STAFF#1 WAS ABLE TO CATCH HIM AND BRING HIM BACK TO THE CLASSROOM. ON 9/15/20 CHILD#1 RAN BACK OUT THE DAYCARE BEHIND INDIVIDUAL#4 AFTER BEING DROPPED OFF. INDIVIDUAL#4 RETURN CHILD#1 BACK TO THE FACILITY. LATER ON THE SAME DAY 9/15/20 CHILD #1 LEFT OUT OF THE CLASSROOM WHILE STAFF#2 WAS TENDING TO ANOTHER DAYCARE CHILD. CHILD#1 WALKED PASSED THE FRONT DESK OUT THE DOOR, CHILD#1 WALKED DOWN A NARROW SIDEWALK OF A BUSY TWO LANE STREET. HE WAS FOUND TWO BUILDINGS AWAY BY A PASSERBY WHO CONTACTED THE POLICE. CHILD#1 WAS FOUND A BLOCK AWAY FROM A CONSTRUCTION LOCATION. STAFF#1 AND STAFF#2 DID NOT REALIZE CHILD#1 WAS MISSING UNTIL THEY WAS RETURN BY THE LOCAL POLICE DEPARTMENT. THE DAY OF COMPLAINT VISIT 9/18/20 OBSERVED CHILD#3 IN THE HALLWAY ALONE. STAFF#3 STATED THEY WERE ASSISTING A DAYCARE CHILD IN THE BATHROOM WHEN CHILD#3 STEP OUT IN THE HALLWAY ALONE WITH NO SUPERVISION.

Correction Required: Children must be supervised at all times. The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for counting children is followed at all times e.g., a checklist. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format.

Noncompliance Area: ON 9/15/20 IT WAS VERIFIED THAT 8PS CHILDRENS WAS IN THE CLASSROOM WITH STAFF PERSON #2 PREPARING FOR NAPTIME. STAFF#4 BROUGHT IN 9PS CHILDREN WHO WAS NOT NAPPING. STAFF#2 LEFT FOR LUNCH WHICH LEFT STAFF#4 WITH 17 PS CHILDREN 8 WAS NAPPING AND 9 WAS STILL AWOKE.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level).

Noncompliance Area: OBSERVE THE EMERGENCY CONTACT FORM FOR CHILD#6 WHICH IS MISSING THE ADDRESS FOR THE PHYSICIAN.

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care.

Noncompliance Area: ON THE DAY OF VISIT OBSERVE THE FILES FOR CHILD#1, CHILD#2, CHILD#3, CHILD#4, CHILD#5 AND CHILD#6 WHICH DID NOT HAVE PROOF OF THE CURRENT INFLUENZA SHOTS.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: ON THE DAY OF VISIT OBSERVED THE FILES FOR STAFF#1 AND STAFF#2 WHICH HAVE PROOF OF 1 NON FAMILY REFERENCE IN THE FILES.

Noncompliance Area: ON THE DAY OF VISIT OBSERVED A SMELL COMING THE UTLITY CLOSET.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
