Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Empower Self-Evaluation invitation was emailed to the provider. The Emergency Disaster Contact Form was emailed to the provider. The DES Group Size was observed in compliance at the time of the inspection.
