The Town of Buckeye is home to one of the finest Recreation Centers, equipped with a large multipurpose room, preschool room, kitchen, and recreational area. The Dr. Robert A. Saide Recreation Center is used for a variety of purposes, including Special Interest Classes, Lil' Squirts Toddler Program, and Teen Activities.

The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but are not limited to: *Use of the outdoor patio *Supervision of visitors (student aides)

Initial Comments: Amended Statement of Deficiencies on 9.30.24 The following deficiencies were observed at the time of the Compliance Inspection conducted on 9/24/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Empower Self-Evaluation invitation was emailed to the provider. The Emergency Disaster Contact Form was emailed to the provider. The DES Group Size was observed in compliance at the time of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *The current license will be posted *The end date of the dosage period on the medication authorization form can be the date school ends for summer break. *Staff files can be maintained in the lobby office as long as they are accessible during operating hours *Electronic rosters (Procare) are acceptable as long as they can be maintained for 12 months. The DES Group Size was observed compliant at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *All unused outlets in the Gym should have a plug insert or cover. *Recommend removing the Emergency Information and Immunization Record cards of disenrolled students from the binder of cards that accompanies students on walking field trips. *Discussed activity room capacity being determined by 50 square feet per child due to the lack of an outdoor activity area.
