The last flu shot on the list of vaccinations in the file for child #3, a 3 year old, was in 2018. None of the files included a written statement of exemption by a parent.

Correction Required: Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Noncompliance Area: The last two health assessments in the file for staff person #1 and #2 were dated more than 24 months apart.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: On the date of inspection, it was observed that the single unit smoke detector system in the rear room was not operable because the battery was dead. This condition at the facility poses a threat to the health or safety of children.

Noncompliance Area: At approximately 11-11:30 am, there were 7 children in the rear room during arts and crafts time, including a young toddler. The capacity for the room is 3.

Noncompliance Area: A small area of the plastic, wrapped around the basement pillars for safety, was peeling off and was low enough to be accessible to the toddlers playing in the child care space.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The initial child service report in the file for child #5 was dated 7 months after the child began attending the program.

The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3280.124(f).
