Facility person #1 has a disclosure statement dated 1/20/2020, a PSP clearance dated 1/20/1010, a child abuse clearance dated 1/202/2020 and an FBI clearance dated 2/20/2020. A request for NSOR certification was mailed 3/13/2020. The facility has a provisional hiring waiver dated 1/1/2020.

NSOR certification was received on 3/25/20 for facility person #1, but last day of employment was 6/26/20. Operator will comply with CPSL and for any future hires I will be sure to request for NSOR certification before they begin working at the center.

Noncompliance Area: When Rep arrived at facility, an infant was observed sleeping in a crib on a boppy pillow in an inclined position. Staff confirmed that the child is under 1 year old. Rep reviewed file and there is no physician's statement regarding a medical reason for a sleeping position other than the position recommended by the AAP.

Correction Required: Toys, bumper pads, or pillows may not be present in a crib while an infant is sleeping in the crib. Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

The boppy pillow was immediately removed from the crib. From this day forward all toys, bumper pads, or pillows will not be placed in a crib while an infant (child under 1 year old) is sleeping in the crib. All staff was reminded of this regulation and a notice was hung in the infant area.

Noncompliance Area: Rep observed a tube of Nystatin cream which is a prescription medication. The medication does not contain an prescription label and instructions from the prescriber are not on file.

Correction Required: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable.

Written instructions from the individual who prescribed the medication was requested and brought in by the parent. In the future we will be sure to have all required information when it is a prescribed medication.
