Inspection Date Reason Description Status
2025-12-17 Renewal 3270.124(f)/3270.181(c) - Updated every 6 months/Emergency info/agreement updated 6 mos Compliant - Finalized

Regulation: 3270.124(f)/3270.181(c)

Description: Updated every 6 months/Emergency info/agreement updated 6 mos

Noncompliance Area: The most recent emergency contact and financial agreement information on file for Child #2, Child #4, and Child #6 were dated 1/2025 and 9/2025. Also, the most recent emergency contact and financial agreement information on file for Child #3 and Child #5 were dated 2/2025 and 9/2025. More than 6 months had lapsed since a parent last reviewed, updated, and signed the emergency contact and financial agreement forms.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Provider Response: (Contact the State Licensing Office for more information.)
The updates were completed when I noticed they were overdue. I will make sure updates are completed at minimum every 6 months.
2025-12-17 Renewal 3270.151(c)(2)/3270.151(c)(2)(ii) - Mantoux TB/Record include X-ray results and evaluation Compliant - Finalized

Regulation: 3270.151(c)(2)/3270.151(c)(2)(ii)

Description: Mantoux TB/Record include X-ray results and evaluation

Noncompliance Area: The health assessment, dated 11/5/25, that was on file for Staff #3 did not contain any results for tuberculosis screening by the Mantoux method at initial employment. The only documentation provided on the health assessment by the physician was a statement that said, "CXR 11/5/25 -neg." Also, there was no record of a positive tuberculosis test or full results of a chest x-ray with an evaluation for chemoprophylaxis.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. If a person's medical record demonstrates a positive tuberculin skin test, that record shall be placed on file at the facility. A record of a person with a positive tuberculin skin test shall include the results of a chest x-ray and evaluation for chemoprophylaxis.

Provider Response: (Contact the State Licensing Office for more information.)
Because the staff was directed by her doctor to get a chest x-ray, and the chest x-ray clearly showed that there was no tuberculosis, I accepted it as the staff was healthy without TB to work in child care, because she was cleared by a Medical Professional. She is currently getting her TB done by the Mantoux Method. If the test comes back Negative, I will forward it to show the results. If the test comes back as positive, as directed by the state, although she has already gotten the chest x-ray, she will get another chest x-ray and the report will be attached.
