A child failed to be signed into the bright wheel app indicating the accurate number of children present. When the workers completed a head count the number of children indicated on the app did not accurately reflect the number of children present. This resulted in a three-and-a-half-year-old child being left outside for approximately two minutes. The workers were not aware of the child's whereabouts until the child was returned to the classroom. Violation
