This results in instances of one or more classrooms being outside while their method of tracking is still inside or vice versa. The system also does not accurately track when a child moves from one classroom to another.

Description: During the investigation of a complaint, staff knowingly provided false information in interviews and written statements to impede the investigation. Text messages obtained during the investigation documented that staff coordinated false information regarding which classroom a child and staff member were in when they became aware that the child had left the building unsupervised. This was confirmed by staff in subsequent interviews.

Description: Based on interview of staff, a parent was not immediately notified of a head injury when child #1 hit their forehead on a climber while coming down the slide. Center's plan of correction from 8/10/20 violation stated that parents would be immediately notified of any head or serious injuries.

Description: The backs of multiple shelving units are not in good repair as several were observed to have large holes in them. The self-closing mechanism is missing from the front glass door, resulting in the door slamming closed with enough force to make it hazardous to a child.

Description: A 2 year old child exited the building alone, passing through at least 3 doors, including a heavy exterior door. The child crossed the center's parking lot and wandered approximately half-way into a large field. A passerby saw the child in the field, with no shoes and did not observe any adults with the child. The passerby pulled into the parking lot and retrieved the child from the field, after which time Staff A exited the building and met her in the parking lot to take the child back into the building. Staff could not identify exactly how long the child had been missing from the classroom.

Description: Upon licensors' arrival at the center, an infant was observed to be crying in an infant swing. The infant cried for approximately 10 minutes without any intervention from staff. The infant was then given a pacifier. The infant was removed from the swing to be fed approximately 5 minutes later.

Description: Per staff statement, injuries observed on a child's face upon the child's arrival at the center were not documented in the medical log.

Description: Based on interview of staff, a parent was not immediately notified of injuries when their child was kicked in the face by another child. Notification was made to the parent several hours after the child was picked up from the center.

Description: Child 1, a four year old child, exited the building without staff supervision when she observed her Head Start bus arriving. Per her statement, Staff A called out to the child to stop, but did not get to the child before she exited, as she was on the floor assisting other children alone as they dressed to go outside. The child was without staff supervision for up to 4 minutes.
