Noncompliance Area: The file for Staff #1 was observed to be greater than 24 months. This is evidenced by the training being done on 9/15/23 and 12/6/25.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and CPR. All staff persons shall renew their certification in pediatric first-aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. One or more facility persons competent in pediatric first aid and pediatric cardiopulmonary resuscitation techniques shall be at the facility when one or more children are in care.

Noncompliance Area: Observed disinfecting wipes and Clorox disinfecting spray solution accessible to children, on a low rest equipment cart in the child care space.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: Observed child file #1, a preschool child, contained an updated health assessment which was dated more than 12 months (7/31/23-8/20/24) after the previous valid health assessment.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Description: Comply with CPSL

Noncompliance Area: Observed staff file #1 contained an updated NSOR verification letter which was dated more than 60 months (12/23/19-3/18/25) after the previous valid NSOR verification letter.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Noncompliance Area: The facility was observed to have an emergency plan that failed to include a statement regarding continuity of operations following an emergency, and to address volunteer emergency preparedness training and practice drills.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

Description: Emergency plan

Noncompliance Area: The facility was observed to have an emergency plan that failed to provide for evacuation of children from the facility building and evacuation of children to a location away from the facility premises.

Correction Required: The facility shall have an emergency plan that provides for evacuation of children from the facility building and evacuation of children to a location away from the facility premises. The evacuation routes and evacuation plans to exit the building may be the same as those required by § 3280.94 (relating to fire drills).

Noncompliance Area: The file for child #1 was observed to contain an emergency contact form that lacked the phone number of the physician or source of medical care.

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care.

Description: Mantoux TB

Noncompliance Area: The file for Staff person #1 was observed to lack a tuberculosis screening by the Mantoux method.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: The facility was observed to lack an emergency drill log that contained the required pertinent information that facilities must use when conducting and documenting the emergency drill.

Correction Required: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility.

Noncompliance Area: The facility's bathroom was observed to contain a blue handled spray bottle which contained a clear liquid but lacked a label that specified the contents.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and childcare spaces.

Noncompliance Area: 2nd citation. Previously cited on 4/6/22. Observed rust on the steps of the monkey bars in the outdoor play space.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The file of Child #2 lacked the work telephone number of the enrolling parent on the emergency contact form. The file of Child #3 lacked the work address and work telephone number of the enrolling parent on the emergency contact form.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent.
