Provider Response: (Contact the State Licensing Office for more information.) I disagree with this assessment. The infant was always in me line of sight through the archway. On a regular day day the crib would have been wheeled out to where the inspectors were but there was no room,or the infant have playing on her blanket with direct supervision. This was not possible with the inspectors in the room, the need to answer their questions, retrieve various binders, and pinpoint the items in the binders. I all times I maintain the proper adult to child ratio. I am not sure if the relevance of this regulation in this context. When this was addressed, the child was taken out of the crib. I will not leave any child in the crib unattended. |

Provider Response: (Contact the State Licensing Office for more information.) I do not see the relevance of having the grownups, work place, work address, or work phone number. I have the cell phone number for all the children's grownups and today that is the best way to reach them in any situation. I will be more meticulous when going over the forms and be sure every line is completed. The day of inspection, I contacted the parents to obtain the necessary information to updated the children's paperwork, |

Provider Response: (Contact the State Licensing Office for more information.) A stroller was added in Attachment 3 to aid in the transportation of infants and toddlers. Attachment 14 was in the emergency plan at time of inspection and was somehow overlooked. The Persons with Special Needs asks for the name, age and special needs of the child. As I do not have a special needs child in my care at this time and this does not feel like something that can be generically filled in. Special needs is a broad term covering many issues including temporary or long term medical situation, developmental delays, physical disabilities, and emotional and behavioral issue. The plan for any of these needs or a combination of would be as different as the child it is for. When I have a child with special needs in my care I will work with the child¿s parents/guardians to complete Attachment 14 making sure to cover a variety of situations and how the child¿s needs could best be met in each instance. I will have the plan completed and forward it to all relevant parties with five days of the child¿s enrollment. I will also send any other changes such as additions to Attachment 3 supplies to be moved. I will keep up to date on the child¿s evolving needs and accommodations checking in with the parents/guardians at least every three months or more frequently if the need arises. If Attachment 14 is updated I will send the update to all relevant parties within five days of the change. |
