2023-02-21 | Complaint | 13A.16.07.03A(1) | Corrected |
Findings: Complaint states that a staff member left a two year old child alone in the bathroom with the lights out and the door closed. Staff member stated that they left the child alone in the bathroom for a minute or so without supervision as a form of child discipline because the staff member was frustrated with the child's behavior. Staff member states that they don't remember if the door was left open or closed. Director and Administrator stated that the camera showed that the door was open. Though short periods of time out is an appropriate child discipline method, leaving a child in a bathroom for time out is not since linking a negative consequence with a physical need is not appropriate discipline. Director stated that corrective measures have been taken. Center shall submit a letter of corrections indicating the corrective actions that have been taken and the results of those corrective actions.

2023-02-15 | Complaint | 13A.16.07.06A | Corrected |
Findings: Center self reported that an infant was left alone in room Infant A during a fire drill. The center did not ensure the safety and security of the child at all times due to lack of supervision of the infant during the unannounced fire drill. Additionally, one infant was moved from room Infant B into room Infant A without any communication between the staff members in each room. This extra child was not signed in to either classroom upon arrival causing the staff in room Infant A to not know that they actually had 7 children in care. The staff in room Infant A only accounted for 6 children in care, which directly led them to leave one child behind in the classroom during a fire drill. Center shall submit a letter of corrections regarding new plans to ensure that this situation does not occur again. Center shall retrain all staff on name to face methods and proper communication between classrooms when moving children from one room to another to ensure safety and security of children at all times.
2023-02-15 | Complaint | 13A.16.08.01B(2) | Corrected |
Findings: Center self reported that an infant was left unattended in a classroom during an unannounced fire drill. The director stated that the staff member who was alone with 3 infants in room Infant B placed a 4th infant who had just come in for the day into room Infant A without notifying the staff in that room of the extra child. The fire drill occurred and the staff in Infant A thought they still had 6 infants when they actually had 7 infants and took 6 infants outside for the fire drill, leaving one infant in the classroom unattended. The staff in Infant A and B were not prepared to provide for the appropriate individual care of the infant who was moved between the classroom and the child left behind in the classroom. The center shall submit a letter of corrections and retrain all staff on methods to provide appropriate care to children, including updated communication methods when moving children between classrooms.

The Teacher in room Infant B placed the infant into room Infant A without communicating with the staff in room Infant A as to how many infants were already in room Infant A. This caused room Infant A to have 7 infants in care. Center shall submit a letter of corrections indicating the plan to ensure that group size maximums are not exceeded.
