Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. In September of 2023, a staff member was observed using an inappropriate tone and foul language when interacting with a one-year-old child. On October 25, 2023, the same staff member picked a one-year-old child up by both arms and roughly sat the child down in a chair.

Diapers were not changed whenever they were soiled or wet and/or were changed on a shift basis. In space #3, it was documented on the diapering chart that a child was changed at 8:05 am and then was not changed until 11:05 am.

Screen time was offered to children under three years of age. In space #4, four (4) children ages 0-1 were in care with one caregiver, and a tablet was observed sitting on a shelf facing the children and playing music videos.

A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. A child enrolled in space #2 had an albuterol inhaler on file without written permission from the parent.

Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. A child enrolled in space #2 and a child enrolled in space #3 had an albuterol inhaler on file that were not stored in the original pharmacy labeled container.

Parent's medication authorization did not include required information. Written permission for an albuterol inhaler in space #3 was documented on a permission form for Craven County Schools.

Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. A safe sleep chart for a child in space #3 was not maintained for the entire time a child was sleeping today from 9am to 11am.

All staff records, except financial records, were not made available for review. One staff member assigned to space #2 did not have a file available to monitor.
