Expired medication was observed in a classroom with children who are three and four years old and was not returned to parents or discarded.

The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. Documentation of a reviewed shaken baby policy for a new staff member, employed in September 2023 was not available for review.

Staff required to receive on-going training had not completed the required number of hours according to their education and experience. A staff member counted in ratio has not completed the required amount of training. This is a repeat violation.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff member has not renewed First-Aid training. The First Aid training expired in December 2021. This violation was cited in the previous June 17, 2022 annual compliance visit.

All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A staff member has not renewed CPR training. The CPR training expired in December 2021. This violation was cited in the previous June 17, 2022 annual compliance visit.

Menus for all meals and snacks were not current or posted where easily seen by parents and cook. Although, this is a boxed lunch school, the provider does serve snacks and the snacks menu was not posted.

Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of three years for the date of issuance). Three staff member's files did not contain updated qualification letters.
