In the transportation book on the bus, photographs were missing for two (2) children that are currently being transported by your facility.

Equipment and furnishings were not sturdy, stable and free of hazards. In space five (5), the door leading to the bathroom had a protruding loose screw which caused separation in the wood. Also, in space five (5) an inoperable bathroom stall was accessible to the children. In space two (2), a strip of wood around the diaper changing sink was separating from the countertop.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space five (5), three (3) electrical outlets did not have covers.
