Equipment and furnishings were not sturdy, stable and free of hazards. In classroom space 11, there was one (1) patch of peeling paint in the bathroom and peeling paint was observed on the bathroom door. In classroom space 13, one (1) patch of peeling paint was observed on the wall near the manipulatives center, one (1) patch of peeling paint and damaged wall was observed behind the trash can in the bathroom and peeling paint was observed on the bathroom door.

Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Departure time for two (2)children in the group with school-age children was not documented on 6/5/23.

Equipment and furnishings were not sturdy, stable and free of hazards. Peeling paint was observed on the walls in spaces #11 and 13.

An outdoor swimming pool located on the premises was not enclosed by a four foot fence, separated from remaining play area by that fence, and locked and inaccessible when not in use. The gate leading to the swimming pool was not locked.

Toxic plants were accessible to children. Poison ivy located near the fence on the toddler playground.

Air conditioning units were accessible or did not have a guard to keep objects from being thrown into the unit. The fenced gate around the HVAC units was not locked. A guard is needed due to the units are located next to the fence and items can be thrown into the vent. This area is accessible to children and used to walk to the playground and for emergency evacuation.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space #1 there was four aerosol cans of Coloration Foam Paint and play several sand bags with cancer warnings was accessible on the playground with the embankment slide. The aerosol cans were removed and this portion of the violation was corrected during the visit.

Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In Space #2 with children under 12 months of age, the staff member stated she did not document safe sleep checks for two (2) infants present on 6/5/23.

All vehicles used to transport children were not free of hazards. Several seats were torn in the blue and red bus used to transport children.

The center did not have developmentally appropriate equipment and materials accessible daily. In group #5, there are piles of unrelated materials in the writing center and game center. Developmentally appropriate materials for the intended use in each center must be accessible to children.
