A safe indoor and outdoor environment was not provided for the children. On the playground used with school-age children, two rubber tires were stacked up under a metal balancing bar to be used as steps. A child was using the metal bar. Another child began removing one of the stacked tires. The child using the bar panicked, letting go of the bar, losing her balance which resulted in the fall. | |||
