A full inspection was conducted. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation was completed at the time of the inspection. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: - Staff files documentation (annual training, new employee training, tuberculosis screening, criminal history affidavits, centralized background checks) - Emergency practice drill documentation - Diaper-changing sinks, water temperature - Diaper-changing procedure posting resources - New rule highlights - Emergency Information and Immunization Record forms - Hours of operation, update procedures

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 10/29/2024, and are subject to changes pending programmatic review. Compliance Officer #1: Chloe-James Rossi Compliance Officer #2: Flossie A. Wagner The Plan of Corrections is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed, but is not limited to: *Ensure food served to enrolled children is not a choking hazard, *Ensure lesson plans are current, *Ensure bathroom storage is in a closed container, *Ensure diapering sink temperatures are maintained between 86°F and 110°, and *Ensure outdoor activity areas are maintained free from hazards. There were 6 staff files reviewed. 6 of the 6 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection. The Emergency Disaster Contact form was completed at the time of the inspection.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 10/26/2023 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The DES Contact form was completed at the time of the inspection. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Ensure gloves are stored on shelf until diaper change. *Ensure parents list two individuals as emergency contacts on Emergency, Information, and Immunization Record.
