Noncompliance Area: In the child care play space there was a cracked plastic school bus.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: A cat in the child care space was observed to be infested with fleas.

Correction Required: A pet or animal present at the facility, indoors or outdoors, shall be in good health and known to be friendly to children.

Noncompliance Area: The files for child #1 and child #2, both preschool aged, contained immunization reports that lacked verification of a flu shot conducted during the previous 12 month period as required for children over the age of 6 months according to the ACIP immunization schedule.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: The file for child #2 does not contain signed parental consent for emergency medical care.

Correction Required: A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.

Noncompliance Area: The file for child #2 does not contain signed parental consent for administration of minor first-aid procedures.

Correction Required: A child's record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Noncompliance Area: Although the file for Child #1 contained a current health assessment, Child #1's initial health assessment is dated more than 60 days after the child's enrollment date.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: A sewing machine was observed on the floor in the dining room. The needle of the sewing machine was accessible to children in care.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Seven children were observed in care during the inspection. Apparently, a school-age child was supposed to be picked up by his school bus prior to the arrival of other children in care. The bus did not arrive as expected. When OCDEL inspector arrived, provider was in the process of having said child picked up by a family member. Child was eventually picked up by a family member during inspection, which brought the number of children in care to six.

Correction Required: The number of children in care may not exceed six children at any one time who are unrelated to the operator.

Noncompliance Area: Observed on the wood picket fence that encloses the outside play space were four nails protruding through the fence facing towards the play space. Nails were in area accessible to children in care.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility`s outdoor play area, shall be kept clean, in good repair and free from visible hazards.
