In the older toddler area of the building at approximately 115pm there were four children on nap equipment in a row next to each other not leaving at least 2 feet of space on 3 sides. In the young toddler area, there were 4 children using rest equipment that did only have at least 2 ft of space on 3 sides.

Per staff statements and other documentation on 3/11/26 at approximately 145pm a tornado warning was issued. Staff and children were evacuated to the basement of the facility. Once a name to face was completed approximately 5-15 minutes after being in the basement it was confirmed that a preschool age child was left up upstairs in the facility alone sleeping behind a table on their nap equipment unable to be seen.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Correction Required: Tiered LIS: Tier 1. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. This portion of the plan shall have an immediate correction date. Tier 2. The legal entity shall develop a visual method for identifying specific children for whom each staff person is responsible and for tracking children during transition periods. All staff will utilize the identification method appropriately at all times while on duty at the facility. The operator shall provide a date for when this will be completed.

Per staff statements, Staff #9 smacked a child on the back of the hand.
