There was not evidence of this happening and teachers were unaware that it was needed.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There were two exposed outlets in the teachers lounge(the door was opened), in Space three(3) there was an exposed outlet by the shelf in the circle time area.

Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. The posted menu stated that children will be served Chicken sandwich, baked beans, apples sauce, and milk. Children were actually served a Chicken sandwich, corn, a banana, and milk.

A safe indoor and outdoor environment was not provided for the children. The playground has a painted chain link fence, the paint is peeling and flacking green paint. There are about five (5) on the three year old playground and thirteen (13) on the Four year old playground.

In the indoor environment in Space D there was an aerosol can not in locked storage.
