Noncompliance Area: The file of staff #1 with a start date in care of children of 4-1-14 had PA State police clearance dated 4-4-14, PA Child Abuse clearance dated 4-25-14 and an FBI clearance receipt dated 4-4-14. The file of staff #2 with a start date in care with children of 4-21-15 had an FBI clearance dated 1-9-16. The file of staff #3 with a start date in care with children of 11-18-19 lacked timely clearances; they were dated: PA State Police 1-31-20, PA Child Abuse 1-31-20, and FBI 12-18-19

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #4 may not work in a child care position at the facility.

Noncompliance Area: Observed in the infant and toddler spaces lacked postings of a daily schedule.

Correction Required: The written plan of daily activities shall be posted in the group space.

Noncompliance Area: The file of child #1 lacked an emergency contact form with the address of the individual designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Observed in the infant space medications not labeled. The medications were as follow: Balmex Complete Protection, Desitin, Parent's Choice diaper rash ointment, Dollar General brand Vitamin A &D. Observed in the older toddler space medication not labeled. The medications were as follow: (2) Desitin and Equate diaper rash ointment. Observed in the young toddler space medication not labeled. The medications were as follow: Desitin and Equate baby diaper rash ointment.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: Observed in the infant space expired medication labeled for child #2 with an expiration date of 4/2019.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Noncompliance Area: The file of staff #1, an aide lacked documentation of education at the time of the inspection.

Correction Required: An aide shall have a high school diploma or a general educational development certificate.A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: Observed in the gross motor space, a damaged tennis ball. Observed in the preschool space, a plastic container labeled 'dinos and safari animals' cracked on the bottom.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Observed a toy present in the crib where child #6, an infant was sleeping.

Correction Required: Toys, bumper pads, or pillows may not be present in a crib while an infant is sleeping in the crib.

Noncompliance Area: The files of children #1, #2, #3, #4, and #5 lacked agreements with the persons designated by a parent to whom the child may be released.

Correction Required: An agreement shall specify the persons designated by a parent to whom the child may be released.

Noncompliance Area: The gross motor space lacked copies of emergency contact forms for children receiving care in the space.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Noncompliance Area: The files of staff #2 and #3 lacked verification of child care experience prior to working with children in care at the facility.
