Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: The facility's current 2025-2026 certificate of compliance was not displayed conspicuously and was not found among the facility's onsite records.

Correction Required: The facility's current certificate of compliance shall be posted in a conspicuous location used by parents.

Description: Emergency plan

Noncompliance Area: Staff #1 informed Cert Rep that there is no Annual Emergency Drill log on file because the facility does not conduct Annual Emergency Drills, as required.

Correction Required: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility.

Description: Plan reviewed/updated annually

Noncompliance Area: The Facility's records did not contain documentation to verify that the Facility's Emergency Plan was reviewed annually, as required. Cert Rep noted that the last documented Emergency Plan Review was dated 1/15/25.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan and kept on file at the facility.

Noncompliance Area: The emergency consent information form on fil e for child #1 did not have parent's written consent to provide emergency medical care and administration of minor first-aid.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care. A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Noncompliance Area: 4 Trash cans were observed in the child care space. The school age program did not have a specific trash can dedicated to the program and the trash cans did not have a lid to prevent contamination by human secretions. A lidded trash can is in line with federal requirements administered by the U.S. Department of Human Services, Office of Child Care (OCC) and its requirement for all school-age programs providing care exclusively to school-age children certified by the Department of Human Services (DHS) under 55 Pa. Code Chapters 3270 and 3280 to follow certain guidelines pertaining to the handling, storage, and disposal of hazardous materials and biological contaminants.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Staff #2 and staff #3 did not have on file a health assessment.

Correction Required: A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: Staff #4 does not have current pediatric first-aid/CPR training. This is evidenced by the previously documented pediatric first-aid/CPR training expiration date of 10/14/24.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.

LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 may not work in a child care position at the facility.
