A second child was observed laying on her mat with a bottle. The second child was also observed walking around the living room area while drinking from the bottle.

Indoor stairs with more than two steps were not made inaccessible to children two years of age or younger. The gate to the indoor steps leading to the second floor was not latched, and was hanging partially open.
