There should have been another exam at child's 2 year birthday. There was no updated health assessment on file for child #3 (DOB 8-12-16). The last health assessment was dated 2-3-17. There should have been another exam 6 months later and then again at child's 2 year birthday.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: At the time of the inspection, there was no update to the emergency contact form and agreement for child #1 and 2(last on file was dated 2-13-18) and for #3 (last was dated 4-8-18).
