Regulation: 3270.113(a)/3270.113(a)(1) Description: Supervised at all times /Staff assigned to specific children Noncompliance Area: During the complaint investigation conducted on 8/15/2025 it was determined that on 8/13/2025 a child eloped from facility. Child went through the gate (push bar), crossed the street and made it to a passing pedestrian. Staff stated that the child was retrieved by staff person #1 within 1 to 3 minutes. Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Tiered LIS 1. Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. This portion of the plan shall have an immediate correction date. 2. The legal entity must develop supervision policy, that includes a plan of action to remain in compliance with supervision when you have children enrolled that display elopement behaviors. Prior to implementation, this policy will be sent to the OCDEL Northeast Regional Office for approval. The legal entity will have a meeting with all staff to review the supervision policy. All staff will sign off on the supervision policy stating they understand it. This will be placed in each individual file. The operator will provide a date for when this policy will be completed and reviewed with staff. 3. The legal entity must arrange for all facility staff to receive a minimum of three hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. The legal entity will place a copy of the sign in sheet from the training in each individual file. The operator shall provide a date by when this will be completed. |

Each employee finished an online training on supervision thru Better Kid Care. Correction date 8/13/2025. 3. OCDEL approved 3-hour supervision training is in the process of being scheduled. Sign was posted to use the side door. Damage to outside glass happened minutes prior to unannounced inspection. Service repair glass company was called immediately. Glass was repaired within 2 hours. There are 2 other doors in which were able to be used during repair. |
