On the day of inspection it was noticed that student 2 had been enrolled longer than 60 days, however we did not have health assessment on file. Student number 2 was withdrawn from the program by the parents do to moving out of our county and the family did not give us a copy of the health assessment before the students last day. In the future all students will be asked to turn in their health assessment forms and immunization records with in 30 days of their start date which is our district policy.

Child 1 did not have a recent flu vaccine at the time of inspection. We reached out to the students family and counseled them on the importance of the flu vaccine, however they choose to sign the flu exemption letter instead of scheduling a flu shot for their child. We have reached out to the parents of Child #3 for a current immunization record, and this will be placed in the child's file. In the future all student immunizations will be checked for the appropriate vaccinations and families will be notified of any missing or late vaccinations. Should a family choose not to have a child vaccinated they must complete a vaccination exemption letter.

On the day of inspection Pre-k Classrooms 13 ad 36 we being utilized. This was a last minute change by the school district to accommodate the need for exterior exits for each pre-k room to limit the amount of cross contamination between classrooms due to Covid19. Due to the late notice the supervisor of child development forgot to contact the state rep to measure and license these rooms prior to the first day of school. The licensing rep was made aware of the reason and change of rooms and measured the rooms on the day of his inspection. In the future the program will contact the state licensing rep and make them aware of any changes to room locations and schedule a day to measure and license the rooms prior to any students attendance.

A school district request to the maintenance department to check the wood chips at each of the playgrounds. The staff at Belle Valley raked the wood chips under the slides to ensure 9 inches of material. If the Maintenance department feels more wood chips are needed they will purchase them and add them to the playground. As always and in the future all areas of the playground will be checked before students use the space to be sure the playground meets all safety requirements.

A meeting was held with the child development employee's to discuss this issue. A substitute was covering with the onsite group supervisor that day and the student whose name was not remembered was new...it was their first day. The substitute knew the student was part of her group but could not remember the students name. In the future all staff will have a list with the names of each of the students in their group, so that if a substitute comes into cover they can quickly call and account for the students in their group.
