Findings: Based on review of staff files, as well as documentation from the Arizona DPS website, the Surveyor determined that Staff 1's Fingerprint Clearance Card lapsed for a short time when it was last renewed. Staff 1's previous Fingerprint Card expired 03/04/2015, and the DPS website documents that the renewal application was received 03/25/2015. Staff 1's date of hire is documented as 11/15/2014, and the current Fingerprint Clearance Card was verified as valid during the inspection, with an expiration date of 04/08/2021.
