|Findings: Based on review of required documents, as well as Provider statements, the Surveyors noted that the facility was not able to document diaper-changes for an enrolled child, Child 3, that was wearing a pull-up diaper during the inspection. *The deficiency was corrected during the inspection*|
|Findings: Based on review of sign in and sign out cards the Surveyor noted the following: -Child 1 was signed in 10/05/2018 and 10/15/2018, but the time out and a signature were not documented on those days. -Child 2 was signed in 10/05/2018 and 10/15/2018, but the time out and a signature were not documented on those days.|
|Findings: Based on inspection of the outdoor play area the Surveyor noted that the wood chips within the fall zones of the plastic slide structure did not meet the minimum 6 inch depth, the resilient material was only 3 inches deep.|
|Findings: The Surveyor observed the following while inspecting the outdoor activity area: -A ladder and a rake that were on the ground. -Between two storage sheds there was a piece of sheet metal with a sharp edge, and behind the sheet metal, there were several boards containing nails.|
|Findings: The Surveyor observed a window-mounted air conditioning unit that is accessible to children in the outdoor activity area.|

|Findings: Based on review of staff files, as well as documentation from the Arizona DPS website, the Surveyor determined that Staff 1's Fingerprint Clearance Card lapsed for a short time when it was last renewed. Staff 1's previous Fingerprint Card expired 03/04/2015, and the DPS website documents that the renewal application was received 03/25/2015. Staff 1's date of hire is documented as 11/15/2014, and the current Fingerprint Clearance Card was verified as valid during the inspection, with an expiration date of 04/08/2021.|
