| Findings: Supervision Concerns. There were concerns this facility had workers on site without proper clearances and were over child/staff ratios. During the investigation, it was found that there was a staff member without CPR and 1st Aid certifications. Additionally, there was no supervision plan in place which left children unsupervised and the number of children onsite was unknown. Resolution: The staff member without CPR/1st Aid will not work until they receive their certifications. Staff members will be rotated during outside play to monitor bathroom breaks and ensure supervision at all times. | |||

| Findings: There were health and safety concerns for children. During an investigation, rust was found in the fridge and the thermometers were not working correctly. Additionally, two (2) swings were found in the baby room. Resolution: The director was educated about health and safety concerns regarding the conditions of the fridge and safe sleep for infants and toddlers.The facility obtained new thermometers and cleaned the rust from the fridge. Additionally, provider removed the baby swings form the infant room to ensure safe sleep was being followed. | |||
