Description: Minimum meals requirements are not always being met. Per a review of the menu for March 2019, breakfasts do not included the required fruit or vegetable component. Also, milk is not being served with lunch and dinner as required. In addition, some meals and snacks do not always contain all of the required components. For example, "assorted chips" are being served with water for snack and "chips" are included as a component for some meals.

Description: A child was not closely supervised to prevent harm and assure safety when staff dumped out a bucket of toys on the floor that included a small green toy and other choking hazards and the child put one in his mouth. Staff only intervened after it was brought to their attention by Licensor Slatten that the child had put a small toy in his mouth.

Description: Food is not being stored at temperatures that protect against spoilage when a bag of chicken wings is observed thawing in the sink.
2019-01-03 250.06(8)(b)4. Furnishings - Sleep Surface - Child Under 1 Year
Description: A 7 month old infant was not provided with a safe sleep surface when he was placed in a Pack N Play that was broken (side would not lock into place) and the mattress was set to the highest level. The infant was observed sitting up in the Pack N Play and he was later observed crawling on his hands and knees on the Pack N Play mattress. In addition, one infant was not provided with a safe sleep surface when there were three children under one in care and only one Pack N Play and one crib was provided.
