| Findings: Supervision Concerns. There were concerns that child had received a head injury. Additionally, there were concerns of the lack of supervision leading to staff being unable to locate children. The health department performed an investigation and determined that staff had not been performing active supervision allowing another child to scratch a child on the forehead. Resolution: The two staff supervising at the time of the incident will be completing TTA Active Supervision and training on writing incident reports. | Findings: Supervision Concerns. There were ongoing concerns of a small child coming home with bite marks on a regular basis. Additionally this child fell from their highchair and was taken to a doctor to assess for stitches in their mouth. Upon investigation, the health department determined that there is a substantial supervision concern in this child's classroom. The investigator observed staff not actively supervising children and confirmed that the child fell out of the chair but did not require stitches. Resolution: The provider agreed to keep more experienced aids in the classroom until the new experienced, lead starts. At a follow-up investigation, the health department observed teachers actively supervising the children and the hiring of a new lead teacher with 20 years of child care experience. Additionally, all staff participated in the Active Supervision TTA training conducted by IdahoSTARS. | |||

| Findings: A serious injury was reported by the child care facility. During the investigation, it was confirmed a child slipped and fell on a slide at the park and hit his tooth. Resolution: The child was comforted and a compress was put on the mouth to stop the bleeding. Parents were called and picked the child up. P rovider implemented new schedule changes to stagger class recess times, and required all staff to take Active Supervision TTA Training to better prepare teachers. | |||

| Findings: Serious Injury. There were concerns a child tripped on uneven pavement while playing outside at recess. During the investigation it was found that the child was taken to the emergency room and needed 8-9 stitches for a gash on their forehead. The health department verified that ratios and supervision were in compliance at the time of investigation and during the time of the incident. Resolution: Provider advised to ask the University to replace the uneven sidewalk to prevent future injuries. SInce ratios and supervision were in compliance during the incident, there is no further resolution needed. | |||

During an investigation, it was determined that a certified teacher was not in a classroom at all times and an aid was left alone with children. | Findings: Sand in Diaper. A child had a rash going up their back and appeared to be from scratching at their diaper. When the child's diaper was changed by the parent, they noticed it was full of sand. Facility states that the child's diaper had been changed since being outside and was logged on their documentation log for changing of diapers. Provider believes that the sand in the diaper fell from their shirt after his diaper was changed. Resolution: Provider believes that the sand came from the shirt and fell after the diaper was changed. The Provider will check for this in the future after a child has played outside. | |||
