A copy of the Notice of Inspection Rights was provided at the time of the inspection. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed, but not limited to: 1. Licensed toilet room maintenance. 2. Posted menu requirements. 3. Monitoring gaps in fencing--not more than 4". 4. New rules discussion (effective: 08-03-2025).

Initial Comments: There were no deficiencies observed at the time of the Compliance Inspection conducted on 10/23/2024 and are subject to changes pending programmatic review. The Empower Self-Evaluation Survey link was sent to the Provider. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection.
