We encourage families to contact the daycare provider directly with any questions or concerns. Reports can also be verified with your local daycare licensing office.
| Inspection Date | Reason | Description | Status |
|---|---|---|---|
| 2026-01-12 | Renewal | 3270.123(a)(7) - Services considered extra | Compliant - Finalized |
Regulation: 3270.123(a)(7) Description: Services considered extra Noncompliance Area: The agreement form on file for Child #1, Child #2, Child #3, Child #4, and Child #5 was observed not to have documentation of services which are to be considered as extra. Correction Required: An agreement shall specify the services which are to be considered as extra. |
Provider Response: (Contact the State Licensing Office for more information.) At the time of the inspection, it was identified that the agreement forms for several children did not include documentation specifying services considered as extra. This was corrected immediately during the inspection by updating the agreement forms to clearly outline any additional services and associated expectations. N/A was marked on all the forms in that field. All affected agreements were reviewed and revised to ensure compliance with the regulation. Updated documentation has been placed in each child's file. |

| 2026-01-12 | Renewal | 3270.77(a) - No peeling paint or plaster | Compliant - Finalized |
Regulation: 3270.77(a) Description: No peeling paint or plaster Noncompliance Area: Peeling paint was observed on the outside door trim of the facility. Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. |
Provider Response: (Contact the State Licensing Office for more information.) During the inspection, peeling paint was identified on the exterior trim surrounding the front door frame. This area is part of the building exterior and not within the immediate interior child-use space. To correct this non-compliance, the issue was reported to the property owner/school administration responsible for building maintenance. A work order/request was submitted for the peeling paint to be properly scraped, sealed, and repainted using appropriate exterior paint. Until repairs are completed, the area will continue to be monitored to ensure children do not have access to peeling surfaces. Documentation of the repair will be retained on file once the corrective action has been completed. |

| 2026-01-12 | Renewal | 3270.94(a)(1) - Every 60 days | Compliant - Finalized |
Regulation: 3270.94(a)(1) Description: Every 60 days Noncompliance Area: The facility's fire drill log was observed to not have done a fire drill every 60 days. This is evidenced by a drill being conducted on 10/20/25 and 1/7/26. Correction Required: The Director or designated staff person who is responsible for compliance with this chapter ensure fire drills are conducted at least once every 60 days. |
Provider Response: (Contact the State Licensing Office for more information.) During the inspection, it was identified that a fire drill had not been conducted within the required 60-day timeframe. Upon notification, the facility immediately addressed this by reviewing the fire drill schedule and ensuring that future drills are conducted within the required timeframe. A fire drill schedule has been re-established to ensure compliance, and documentation procedures have been reinforced to accurately reflect each drill conducted. |
