Noncompliance Area: During the electronic paperwork review the Rep observed that facility person #1 received emergency plan training on 8/22/19. Facility person #2 received emergency plan training on 7/17/19. Facility persons #3 & #4 received emergency plan training on 7/1/19. This training is more than 1 year old and has not been updated.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: During the electronic file review the following was observed: Facility person #5 began working in child care on 3/15/20 and NSOR certification was not requested until 6/2/20 and is dated 7/28/20. Facility person #6 began working in child care on 12/9/10 and NSOR certification is dated 8/4/20 with no proof of request on file. Facility person #7 began working in child care on 11/26/19 and NSOR certification is dated 6/9/20 and was requested 6/8/20. Facility person #8 began working in child care on 12/6/19 and NSOR certification is dated 6/8/20 and no proof of request is present. Clearances other than the NSOR were obtained properly for facility persons #5- #8. Facility person #9 began working in child care on 2/4/20. PSP clearance is dated 12/6/18, child abuse clearance is dated 2/12/20 and was requested 2/3/20, FBI clearance is dated 2/21/20. No other proof of fingerprinting is on file. NSOR certification is dated 6/4/20 and was requested 6/2/20. Facility person #5- #9 were hired provisionally. The facility was granted a provisional hiring waiver on1/8/20.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: During the in-person inspection on 10/5/20, Rep observed a staff person place a bottle of sanitizer on a low shelf that was accessible to young toddlers present. Rep also observed a tube of A&D ointment in an accessible, covered plastic bin on the floor in the bathroom in the green preschool room. The tube is labeled "keep out of reach of children" and was accessible

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: When Rep entered infant room, three staff were present and a child was observed sleeping in a swing. Rep observed the room for a few minutes and when staff did not attend to the child, Rep instructed staff to remove child from the swing and place child in a pack & play. Staff identified the child as a 3 month old.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: Rep observed an infant sleeping in a pack & play with a Boppy pillow. Staff present report that the child is 9 months old.

Correction Required: Toys, bumper pads, or pillows may not be present in a crib while an infant is sleeping in the crib.

Noncompliance Area: Child #3 is missing health insurance policy number on the emergency form.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: Child #2 is missing addresses for release persons.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Child #1 is 7 months old and has only 2 doses of DTAP and 2 doses of pneumococcal vaccines. This is not in accordance with ACIP recommendations.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: Facility person #1 does not have results of TB screening at initial employment on file at facility.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: Child #2 has insurance information on the emergency form in the master file but health insurance information is not updated on the emergency forms located in the classroom resource binders.

Correction Required: If emergency information is updated in a master file, it shall be updated accordingly in other facility records.

Noncompliance Area: Facility persons #2 & #3 do not have acceptable verification of previous experience on file. Experience forms are not signed by a representative of the experiential entity.

Correction Required: A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.

Noncompliance Area: The first aid kit in the pink room is missing gloves and tape.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.
