A safe indoor and outdoor environment was not provided for the children. On the younger playground in front of the stage their are 4 deep holes there are considered tripping hazards for the children when playing on or around that area.

Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. There were plastic bags and bleach water bottles in space # 1, space #2 and space #3 less than 5ft and accessible to children. In space #3 a bogg personal bag with medication was on the floor under the desk and accessible to the children.

Toxic plants were accessible to children. Poison Ivy plants are accessible to children around the fence areas facing the back of the playground on the younger and older playground.

Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence was less than 4feet due to the build up of mulch, broken slats, and panels leaning.

Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member did not complete First Aid within 90 days of employment. Enhanced staff/child ratios and group sizes were not met. In space #4 there was one teacher that had 4 four year old children and 11 five year old children, which exceeded their ratio size.
