Noncompliance Area: Child #1 has an initial health report that is not signed by a physician and does not contain a statement that the child is free from communicable disease and able to participate in child care.

Correction Required: A health report must be written and signed by a physician, physician's assistant or a CRNP. The signature must include the individual's professional title.A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease.

Noncompliance Area: Rep observed a bin of sunscreen bottles on a low table near the door to the playground in the preschool room. This is accessible to children. Sunscreens are labeled "Keep out of reach of children." Operator reports it is applied to children as they go outdoors and otherwise kept in an inaccessible area.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: Children #2, #3 & #4 do not have release person addresses on the emergency information form. Child #4 also does not have release person phone number on form.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Operator's parent handbook contains means of transporting a child for medical care but that information is not posted in child care spaces or in the travel bags.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: Child #1, age 7 months, is missing doses of Rotavirus, DTAP, Pneumococcal, polio and influenza vaccines. Children #2, #4, &#5 are missing current influenza vaccine.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: Rep observed several expired diaper creams in the toddler changing area.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Noncompliance Area: Safe pick up and drop off points are included in the parent handbook but are not posted in the facility.

Correction Required: Written notification of safe routes shall be posted by the operator at a conspicuous location in the child care facility.

Noncompliance Area: Operator is currently logging injuries and notifying parent but is not providing parent with copy of injury report or placing copies in the child's file or center file.

Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

Noncompliance Area: Observed holes on the plastic covering of the blue & green couched in the preschool/school age room. Also observed ripped clear plastic covering on soft blocks in the infant/toddler room.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Child #2 does not have any information recorded for health insurance on the emergency information form. It is unknown if child lacks insurance.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: Child #1 does not have address for a release person on the emergency information form.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Observed a DHS medication log that contained a parent's signature but did not contain the name of the OTC medication (Children's Motrin) or instructions for administration and dosage information provided by the parent. The medication had been administered by staff to child on one occasion, therefore it cannot be determined if parent gave consent for the medication administered.

Correction Required: A parent shall provide written consent for administration of medication or a special diet.

Noncompliance Area: Observed children's lunches brought from home not refrigerated. Some of the lunches contained perishable food and cold packs.

Correction Required: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated.

Noncompliance Area: Observed infant bottles that were not labeled with children's names.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: Facility person #1 is employed as an AGS and has a high school transcript that shows she completed early childhood development classes but it does not contain information verifying 400 clock hours of classroom training and 200 hours of supervised training in a child care facility.
