| Initial Comments: The following deficiencies were observed at the time of the Initial Compliance Inspection conducted on 5/20/2026 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. Please send pictures of the following: The back playground after removing the large wood barrier. The mounted toilet paper dispenser in Toilet Room #2. The shade structures in the front Playground area. The new diaper changer surface. The Notice of Inspection Rights was provided to the licensee at the time of the inspection. The fingerprint clearance cards for 1 of 1 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but are not limited to: Children’s sign-in and out records Children's Emergency Information Cards and Immunization Records Rosters Staff attendance records Child abuse and neglect- mandated reporter Staff Qualifications and Files Yearly annual training hours Accessible drinking water Monthly fire drills Ensure the facility is free from hazards All chemicals and toxic items are inaccessible to children Notification of pesticides Written approval for personal product supplies by the facility Specific food on the menu Children with special needs and in the individualized plan Discipline and guidance procedures Written accident, evacuation, and emergency plans Notifying parents and documenting injuries Illness and infestation log Medication procedures Transportation and Field Trip Procedures Notify the Department of the closure for any days Interview the rights of staff and children | |||
