Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: Upon inspection of facility fire safety records, it was found that the facility had on file a fire detection system inspection report dated 3.7.25 from Monitronics which did not include passable inspection results related to the alert system [i.e., horns, strobes, sirens, etc] within the childcare portion of the building.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. §  1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met.

Noncompliance Area: Upon inspection of each childcare space, it was observed in the infant room that one crib was in use with a sleeping child with 2 feet of space on only two sides.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Description: Refrigerator

Noncompliance Area: Upon inspection of the physical site area and equipment for each childcare space, it was found that there was no thermometer in the infant space thermometer.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: Upon inspection of the children's files, it was found that the document on file representing a health assessment [i.e., visit summary] did not include a signature and the individual's professional title of the individual completing the assessment for Child #2 #3 [initial] and #4.

Correction Required: A health report must be written and signed by a physician, physician's assistant or a CRNP. The signature must include the individual's professional title.

Description: Immunization record/Exemption documentation from parent/guardian

Noncompliance Area: Upon inspection of children's records, it was found that Child #2 was missing 2/4 doses of Pneumococcal vaccine and 1/3 doses of Polio vaccine. It was also found that Child #3 was missing 1/3 doses of HIB vaccine and 1/4 doses of Pneumococcal vaccine. Finally, it was found that Child #5 was missing an entire vaccination record.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: Upon inspection of the physical site area and equipment for each childcare space, it was found that there was prescription medication accessible to children in care, in a backpack found in the cubby area of the older toddler space.

Correction Required: Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children.

Correction Required: An operator shall notify local traffic safety authorities annually in writing of the location of the facility and the program's use of pedestrian and vehicular routes around the childcare facility.

Description: General Health and Safety/Locked or inaccessible

Noncompliance Area: Upon completion of the inspection of the physical site, it was found that a utility closet with hazardous/cleaning products [marked 'keep locked'] in the entry area near child restrooms was closed but unlocked. In addition, it was found that a bleach solution in a spray bottle was accessible to children in the bathroom of the Preschool 2 childcare space.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: Upon inspection of the physical site area and equipment for each childcare space, it was found that there were multiple electrical outlets in the Preschool 2 childcare space that were broken and/or improperly attached to the wall.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Upon inspection of the facility emergency plan, it was found that the written plan on file did not contain specifics concerning the children's life-saving medications that were present at the facility and their necessary accommodations.

Correction Required: The facility shall have an emergency plan that provides for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions.

No valid exemptions were on file for either child.

Correction Required: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: During an inspection of required facility child tracking forms, it was found that the facility was out of regulatory ratio in the toddler 1 room on 5.20.22 from 1300-1500. During that time, there was one facility person with between 7-10 children.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan and kept on file at the facility.The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.
